In NY, by completing a Health Care Proxy, you can appoint a person you trust to make health care decisions for you, if you lose the ability to do so. You can obtain a free Health Care Proxy form & information from the NYS Dept of Health website. A free form can also be obtained on the NYS Bar Association website.
Your funeral and burial instructions can be stated in a properly drafted will that is prepared, signed & witnessed in accordance with the requirements of NY law.
