What form do I use to convey my life estate deed back to the grantor?

I was granted a life estate from my nephew and I would like to grant it back to him. We are both in agreement., what is the correct form to use.

One that is prepared by an attorney.  I am not familiar with a set form for such a conveyance (as you will need to do an agreement that is particular to your transaction) but the deed to convey is standard although the language should be tapered.  Good luck.
