I am so sorry for your loss. If your state allows the affiant - you - to file a small estate affidavit then they probably have a statute governing its content. What you need to do is to go to the probate court and see if they have a for that they can give you or go on line to your state court web site and see if the courts have forms for a small estate on line. Generally you have to state the the value of the assets of estate is below a certain amount (the amount set by statute) and probably state the liabilities of the estate, that the funeral bill was paid, etc. The it most likely needs to be signed in front of a notary.
