Need to know what form(s) to file to have ex-wife removed from Title/Owner of the house I have assumed financial responsibility for.

I am getting a divorce and have paid my soon to be ex-wife half of the equity in our home. I refinanced the home and the loan is solely in my name. I need credible advice on what papers to file to have ex-wife removed from title or ownership of the house. Thanks!

You can't force her unless the divorce decree requires her to give up title to you.  If that is the divorce decree, then usually it is called a quit claim deed.  It transfers titles without the warranties in a deed.

Did you have a divorce attorney? Talk to your divorce attorney.  If it was part of the divorce decree, file a motion with the court if she is in contempt and doesn't transfer title.
