If my green card has my maiden name and date of birth across the front but my marriage signature at the bottom, do you think it will be a problem to travel back to Jamaica?
Or do I have to travel with all my documents such as my marriage certificate?

If your passport has a different name than your green card, i would strongly suggest that you travel with your marriage certificate as well as try to get your passport changed to reflect your new married name with the Jamaican Embassy in the US.
