The first thing I'd want to know is the reason being given for this.  It may be that there is something in the health insurance plan, that comes from the insurance company, that is creating this problem.  Benefits like health insurance are covered by a federal statute, ERISA, and there are very technical requirements that have to be followed -- otherwise, the employer's part of the cost isn't tax-deductible!
