Is it possible to get my job back if I falsified a document and a policy explaining the proper way to document came out after the fact?

There was no policy explaining the proper way to document the form I submitted until after I had already submitted the form and was in trouble.

Even if there was no policy on how to "document the form," falsification of a document is never acceptable anywhere.  That aside, Arizona is an "at will" employment state, meaning anyone can be fired for any reason except a discriminatory one. It does not sound like you were discriminated against, so you would not have a legitimate case for wrongful termination in Arizona.
