The divorce decree was incorporated in to an order that was signed by a Judge, correct? Well he is in contempt of that order so you can bring a motion back in front of the Judge that issued the order for contempt and to compel him to execute the paperwork. Judge's do not like it when parties ignore their directives and there are very serious consequences that could be had here (like jail time). Good luck.
