I’m an independent contractor. Am I legally obligated to provide demographic information?

I have an LLC and independently contract with a community college. They want me
and all other people in my same position to fill out a new form after I’ve
been hired and working. The form asks for birthday, address, Social Security
number, and demographics info. Since the college is contracting with my LLC, I
shouldn’t have to give them my personal information, right? I’ve already filled
out their hire paperwork.

The answer is, you have to provide whatever information a person or entity you want to work for requests; if you don't provide the information, they are free to not employee or hire you or provide you work, the same as you are free to walk away from the job or position if you don't want to provide the information.
