There is no specific requirement that they get you medical attention first: it is a judgment call on the part of the police (e.g. how urgent do your needs look?). Even if the police *should* have gotten you medical attention first but failed to do so, and that caused you some injury due to the delay, that would not invalidate the DUI test--it might give you grounds to sue for compensation, however.
