If the man is not divorced from his wife and gets a girl pregnant, does the pregnant girl have to put the father’s name on the birth certificate?

Or even give the child his last name?

The girl can complete the form in any of the ways that you mention.  She can list the child's last name the same as her's or dad's.  She can list a "dad" or leave the section blank.  If she does list man as "dad" then he will be the presumed father of the child.  It's just really up to what the parent or parent(s) decide to name the child.
