If yo make any changes to your Last Will and Testament you are in effect making a new Will and yes, it needs to be executed in front of at least two witnesses and signed by the witnesses.  If your state requires that the witnesss signatures be notarized then they must be so notarized on the new document.  It is a rarity that the New Will have to be recorded.  Please get legal help.  Otherwise you run the risk of it being challenged as invalid. Good luck.
