If I had a case in one state then I must contact a lawyer in that state or in the state in which I live?

I now live in PA but had a surgery 2 years ago in NY.

A lawsuit can be filed where the plaintiff resides or where the defendant resides or where the claim arose.  You could file your lawsuit in either PA or NY.  For convenience purposes such as filing documents with the court and court appearances, it would be advisable to file your lawsuit in PA where you live.  You can hire a PA medical malpractice attorney.
