Is it legal for an employer to require a doctor’s note for using the restroom more than 3 times a day?
There are also several other issues with the company that don’t seem quite legal. What can I do about this?

Having a doctor's note for you to use a restroom by your employer more than three (3) times a day is not illegal. Possibly your employer is concerned that you may have a health issue and is being diplomatic about it.
Hence by asking you to get a note from your doctor regarding your use of the restroom more than three (3) times during the day may cause the doctor to ask you questions about your health and do some tests upon you.
There is nothing you can do about the request other than get the note or not get it.
