Since you were injured at work, you can file a workers' compensation claim.  Your employer's human resources department will have the forms you need to file the workers' compensation claim.  Workers' compensation will pay your medical bills and compensate you for pain and suffering.

Since your current neck problems are due to your work-related injury, you are eligible to file a workers' compensation claim.  If you have any problems with the company attempting to deny your claim due to the amount of time that has elapsed since your original injury, it would be advisable to speak with a workers' compensation attorney.
