What to do if I was physically threatened, sexually harassed and verbally abused by a co-worker?

The manager witnessed most of it and had to physically hold him back from me (I am a woman). After telling me he would be fired, they went back on their word and said he is only suspended. I walked out, but have not officially quit (this happened today). Should I file a police report? If I don’t go back, can I file for unemployment? Do I have a legit case?

If I were you, I would file a police report about the incident and then have a meeting with your human resources department about what happened in order to establish a record over what happened. Make sure to get in writing what will be done to rectify the problem so as to not have it happen in the future.

It is not advised that you quit. If you quit, you might not get unemployment benefits. You have a case against the co-employee for possible damages but not your employer at this point in time.
