If I recently had a major surgery, do I have a case if my manager went and disclosed exactly what my surgery was for to several co-workers?

I informed my manager exactly what it was for but told her not to tell anyone other than management if she needed to since I would be out of work for a while and would have some lifting limitations. Is what she did legal and what are my options if it isn’t.

You can sue your manager for invasion of privacy.  Invasion of privacy in your case is the public disclosure of private facts.
Your damages monetary compensation you are seeking in your lawsuit for invasion of privacy would include mental distress.  If a wrongful motive for the manager's disclosure is established, you may be awarded punitive damages.  Punitive damages are a substantial amount to punish wrongdoing.
