I would recommend that you consult with an attorney about drafting up the grant deed and having it recorded with the country recorder's office where the property is located. If you and your mother decide not to use an attorney there are forms online for grant deeds to review or in your county's law library.

You then use a form as a template, type out the legal decsription of the property and assessor parcel number and have your mother sign it before a notary public. She needs to be aware as to how much interest she is giving to you in the document. The document is then recorded with your county's recorder's office.
