Real or personal property titled in the name of the decedent (your Mom) must go through probate in order to be titled in the name of those that would inherit it. If she died with out a Will then she died intestate and the intestacy laws in Tennessee will govern who inherits the assets. Are you an only child? If you have siblings then they could also have a right to the assets as well. I would try and speak with a legal service agency that help those with disabilities to see if they can help you figure this out. You may need a lawyer. Good luck.
