When should an insurer be liable for hospital charges?

My health insurance policy coverage stated “through” 9/17/”. I purchased an extension starting 9/18 and had an emergency visit on 9/17. My insurer refused to pay stating that my policy expired at 12:01 am on the 17th. However, nothing was provided in writing as to the time of expiration.

Based upon what you have written concenring your insurance polict and coverage through September 17 it would appear under the language that you submitted that any charges you occurred on the 17th would be covered under your insurance policy where your carrier would be obligated to make payment.
